Non-Medicaid Care Management

The primary goal of Non-Medicaid Care Management is to provide person-centered, youth and family driven care to individuals 0-21 diagnosed with a serious emotional disturbance who do not qualify for Medicaid. Services include comprehensive care management, care coordination and health promotion, comprehensive transitional care, individual and family support, referral to community and social support services and use of health information technology to link services.
